About This Program
Sequence MD, Inc. (“SequenceMD,” “we,” “us,” or “our”) operates a recurring text message (SMS) program for patients of our Denver, Colorado and Oklahoma City, Oklahoma clinics.

Messages You Will Receive
After you opt in, you may receive appointment reminders, appointment confirmations, scheduling and rescheduling notices, and general clinic notifications such as office closures. We do not send marketing or promotional text messages through this program. We do not send genetic test results or other sensitive medical information by text message.

How You Opt In
There are two ways to opt in, and both require you to take a deliberate action. First, you can check the text message consent box on the contact form at https://sequencemd.com/. That box is separate from every other consent on the form and is unchecked by default. Second, if you are referred to us by another provider, you can sign the Text Message Consent Form included in your new patient packet, which is also available at https://sequencemd.com/sms-consent.pdf.
Opting in is optional. Consent is not a condition of receiving care or any other service from SequenceMD. If you do not opt in, you can still book and receive care, and our staff will contact you by phone or email instead.

Cost
Message and data rates may apply. SequenceMD does not charge you for these messages. Charges, if any, come from your mobile carrier under your own plan.

How to Opt Out
Reply STOP to opt out at any time. We also accept CANCEL, END, QUIT, UNSUBSCRIBE, and REVOKE. You will receive one final message confirming that you have been unsubscribed, and then no further messages from this program. Opting out of text messaging will not affect your ability to receive care at SequenceMD.

Supported Carriers and Delivery
The program is supported by major US mobile carriers. Carriers are not liable for delayed or undelivered messages. Delivery is not guaranteed and can be affected by your carrier, your device, and your coverage.

Your Privacy
Our Privacy Policy explains how we handle your information and is available at https://sequencemd.com/privacy-policy/. Text messaging originator opt-in data and consent are excluded from all sharing categories described in that policy; this information will not be shared with any third parties or affiliates for any purpose.

Protected Health Information and Safety
Text messages are not a secure channel. We limit the content of text messages accordingly and will not send test results or other sensitive clinical information by text. Please do not send clinical questions or sensitive health information by text. Contact the clinic by phone or through the patient portal instead. If you are experiencing a medical emergency, call 911.

Eligibility
You must be 18 or older and be the account holder or an authorized user of the mobile number you provide.

Changes to These Terms
We may update these Terms. The current version will always be posted on this page with an updated Effective Date.
